Grooming Services Policy Acknowledgement Form
Thank you for choosing the Dog Gone Clean (DGC) family! To ensure the best possible experience for both you and your pup, we require you to acknowledge the following terms before grooming services. This agreement helps us maintain a safe environment, manage expectations, and provide the high-quality care your furry friends deserve.
1. Health & Vaccinations

Rabies Requirement: Proof of updated Rabies vaccination must be submitted via text or verified by your veterinarian via phone at least 24 hours prior to your appointment.

Waiting Period: Any vaccinations must be administered at least 24 hours before any grooming services are performed to ensure your pet has no adverse reactions.

Disclosure: Owners are responsible for notifying DGC of any health issues, physical concerns, disabilities, or allergies during the booking process or at the time of drop-off.

Pets in Heat: In the event your pet is in heat and/or is on their menstrual cycle, we may have to reschedule your appointment. Please contact us as soon as possible to determine the best course of action necessary to make scheduling adjustments.
2. Behavior & Medication

Medication: If your pet requires prescribed medication for grooming (e.g., mild sedatives), it must be administered exactly according to your veterinarian's instructions and the bottle label.

Anxiety & Safety: If a pet is extremely anxious on the grooming table and is not medicated, our groomers may recommend a vet consultation for future visits. We prioritize building trust and rapport with your pet, but we will not compromise the safety of the animal or the groomer.
3. Grooming Expectations & Documentation

Consultation: Customers are encouraged to be specific with grooming expectations. Our groomers will assess the coat type and condition and advise on the best options.

Photo Policy: For quality assurance and to track your pet's progress, every dog will receive a "Before and After" photo.

Mid-Service Changes: If issues arise during the service (e.g., skin conditions or severe matting) that prevent the expected cut from being completed, we will pause the service, take photos, and call the "pup parents" to discuss the best course of action.

We do not provide any flea treatments or services for pets that have an active infestation of fleas, this needs to be treated by your veterinarian prior to grooming appointment. If discovered during service you will be notified. Minor presence of fleas and no active infestation will be treated at an additional fee $10-$15.
4. Fees & Cancellations

Matting Upcharge
Additional fees apply for matting due to insufficient brushing. If discovered during the groom, we will provide photo evidence and contact you.

Late Pick-up Fee ($10)
DGC is not a boarding facility. We call 15-20 mins prior to finishing. Fees apply if pickup is more than 30 mins past the ready time (1 hour total from call).

Cancellation Fee ($25)
Charged for "No Shows" or cancellations made less than 24 hours prior to the appointment.

Late Arrival Fee
If you arrive 15 min past your scheduled appointment time this will be considered a “No Show” and fees may apply. Communication is highly encouraged. If you are running late, please contact us as soon as possible, and our staff will do their best to accommodate you if our schedule allows.
5. Satisfaction & Refunds

Resolution: If you are unsatisfied with a groom, please notify us within 24 hours of the appointment. Our groomers will happily work to resolve any concerns.

Refund Policy: Monetary refunds will not be issued. We are committed to resolving issues through service adjustments.

By signing below, I acknowledge that I have read and agree to the policies listed above. I understand that these policies are in place to protect the well-being of my pet(s) and to ensure a positive experience for all clients and staff at Dog Gone Clean.
*ONCE GROOMING APPOINTMENT IS SCHEDULED, A LINK WILL BE SENT FOR A REQUIRED SIGNATURE OF ACKNOWLEDGEMENT.
